Box 14 is where taxable fringe benefits are summarized, but it does also include a documentation of several Non-Taxable items (i.e., via pre-tax dollars), such as: code K (non-taxable dental/vision deductions); code V (non-taxable health benefits); code Y (non-taxable flexible spending accounts [FSA]).
Health Plans If an employer pays the cost of an accident or health insurance plan for his/her employees (including an employees spouse and dependents), then the employers payments are not wages and are not subject to social security, Medicare, and FUTA taxes, or federal income tax withholding.
Box 14 is used to report amounts that dont belong in other boxes on Form W-2. Employers can use it to report additional tax or income information for filing or informational purposes. Some employers use box 14 to report amounts deducted for State Disability Insurance taxes or union dues that may be tax deductible.
Code DD is only information to you to tell you how much your employer spend for health coverage - you do nothing with it. Code D is the amount of salary deferrals to a 401(k) plan. You do nothing with than either other than enter it on the W-2 screen in box 12 just like it is on the paper W-2.
Box d: Control number. This is a code that identifies this unique W-2 Form document in your employers records. It is an optional field and may be left blank. Note that this number is used to upload your tax information into tax preparation software.
The Appropriated Fund Profile (D22) screen is used to identify appropriated funds and dedicated general revenue (GR) accounts. These funds exist by order of the Legislature, by Comptrollers office policy or for accounting or reporting purposes.
Examples of items that may be reported in box 14 include: The lease value of a vehicle provided to an employee. A clergy members parsonage allowance and utilities. Charitable contributions made through payroll deductions.
Multiply total cost of health care per month by the number of months the employee was enrolled in the plans for that tax year. Report the total in box 12 on the W-2 form using code DD.
