Two acceptable methods for disposing of paper records containing PII are using a cross-cut shredder or placing the paper(s) in a burn bag. Do not use a recycle bin to dispose of paper records containing personal information / PII.
Remember, HIPAA does mandate that unused media containing PHI be adequately destroyed, and not simply left behind or disposed of in a public receptacle. Examples of destroying hard copy include, but are not limited to, burning, shredding, or pulverizing.
Shred Paper Records Paper records containing personal and sensitive information should be shredded with a cross-cut, diamond-cut or confetti-cut paper shredder (strip-cut pieces can be put back together).
The ideal method for eliminating the risk posed by PII/SSNs is to securely delete the files containing them. Securely deleting a file differs from regular deletion in that it overwrites the physical area of the disc where the bytes comprising the file were stored.
How to Securely Destroy Hard Drives: Clearing. Clearing removes data in such a way that prevents an end-user from easily recovering it. Digital Shredding or Wiping. This method does not alter the physical asset. Degaussing. Physical Destruction. Built-In Sanitization Commands. Physical Destruction or Encryption.
