A tunnel diameter greater than 15 mm will require two-stage surgery when the original tunnels are in anatomic position, while revision with a tunnel diameter of less than 10 mm can be accomplished in a single surgery.
If they are larger than 12 to 14 mm in diameter, or are in a position where an anatomic positioning of the ACL reconstruction graft would break into the previously malpositioned tunnel, then a two-staged surgery would be recommended.
It can be concluded that to decrease the risk of having a failure in hamstring ACL reconstruction, a graft of at least 8.5-mm should be used.
Patellar Tendon It attaches from the knee cap to the shin bone. It is the graft with the longest track record and is consider by many to be the gold-standard for ACL reconstruction. In most circumstances this is the graft recommended for professional athletes and in any young active individual.
The ACL measures 31-38 mm in length and 10-12 mm in width, with the anteromedial bundle (6-7 mm) slightly thicker than the posterolateral bundle (5-6 mm) 5.
Graft size less than 8 mm in a young patient less than 20 years is said to be commonly associated with revision surgeries. Graft size 8 mm is said to be associated with a better outcome of the ACL-R surgery and prevents the need for a future revision of ACL-R [20-22].
