In Arizona, a Fingerprint Clearance Card issued by the Applicant Clearance Card Team (ACCT) at the Department of Public Safety is valid for six (6) years. Submitting a renewal application approximately six months before the cards expiration date is recommended to ensure a seamless transition.
If there are any arrests on your record, DPS compares the criminal offenses with a list of offenses that would cause the denial or suspension of a fingerprint clearance card, such as assault, theft, drug offenses, as well as many others. Offenses that appear in this list are referred to as precluded offenses.
You will need to submit a Fingerprint Clearance Card Replacement Request form. The form may be obtained by one of these two ways: Online - A replacement can be obtained online on our Public Services Portal linked below. By Phone - You may call (602) 223-2279 and request the form be mailed to you.
The main difference between a fingerprint card and a fingerprint clearance card is that a fingerprint card is simply a form that collects fingerprints, while a fingerprint clearance card is a document that verifies that the individual has been cleared of any criminal history.
Please contact the Department of Public Safety Criminal History Records Unit at (602) 223-2222 to obtain a fingerprint card, copy of your criminal history record, a Review and Challenge packet, or you can download the information from the Criminal History Records Unit section of the DPS website.
There are two options in acquiring a replacement fingerprint clearance card: By Phone You may call (602) 223-2279 and request a replacement card form be mailed to you. Complete the form and mail it to DPS with the $5.00 required fee (cashiers check, money order or check drawn on a business account).
The most common reason fingerprints are rejected is for the quality of the print. You must complete the reject request and reprint, if you do not your application will remain deficient, and you will not be licensed.
What is the fee to apply for a Fingerprint Clearance card and what forms of payment are accepted? Paper Application - Regular (Non-IVP) or IVP: The DPS fee* is currently $67.00**, except for volunteers. The fee for volunteers is $65.00**.
