Cardiac enzymes ― also known as cardiac biomarkers ― include myoglobin, troponin and creatine kinase. Historically, lactate dehydrogenase, or LDH, was also used but is non-specific. Cardiac enzymes are released into the circulation when myocardial necrosis occurs, as seen in myocardial infarction.
Cardiac Biomarkers (Blood) Cardiac troponin. This protein is by far the most commonly used biomarker. It has the highest known sensitivity. Creatinine kinase (CK). This enzyme can also be measured several times over a 24-hour period. CK-MB. This is a subtype of CK. Myoglobin. This is a small protein that stores oxygen.
When an injury to the heart occurs, it releases certain enzymes. Doctors measure cardiac enzymes to determine whether a person is having a heart attack, including troponin T (cTnT) and troponin I (cTnI). Doctors will typically test for both cTnT and cTnl as these are the most specific enzymes to a heart attack.
If you go to hospital with chest pain or a suspected heart attack, doctors may do a blood test to find your troponin levels, also known as cardiac enzymes. If they find higher than normal levels, this can be a sign that your heart has been damaged.
For troponin concentrations 0.40 ng/mL and higher, the underlying cardiac injury is usually a myocardial infarction.
Since myoglobin is also present in skeletal muscle and is cleared almost exclusively through glomerular filtration, myoglobin will increase after acute muscle trauma, acute or chronic kidney injury, severe heart failure, prolonged shock, and in patients with myopathies of a variety of causes.
