An intact ACL has a mean length of 38 mm (range, 25 to 41 mm).
The anterior cruciate ligament (ACL) consists of two major fiber bundles, namely the anteromedial and posterolateral bundle. When the knee is extended, the posterolateral bundle (PL) is tight and the anteromedial (AM) bundle is moderately lax.
The ACL measures 31-38 mm in length and 10-12 mm in width, with the anteromedial bundle (6-7 mm) slightly thicker than the posterolateral bundle (5-6 mm) 5.
In studies of adults, reports of average ACL length, cross-sectional area, and volume ranged from 26 to 38 mm, 30 to 53 mm2, and 854 to 1,858 mm3, respectively. Reported values were commonly found to vary ing to sex and skeletal maturity as well as measurement technique.
Indeed, the graft needs to be around 70 mm long, with 20 mm on the femoral and tibial sides and 27 to 42 mm in the joint space. An adjustable Pullup cortical suspension device on each side allows the surgeon to adjust the graft length in the tunnels and to manage graft tension.
