Make sure each employee write-up form includes: The employees name or ID Number. The employees position. A specific recounting of the offense committed. The type of warning or discipline enacted. A plan for improvement. Any and all managers or decision-makers who should be aware of the situation.
Body of the Letter: You should explain why you are writing in the first paragraph. Include details about the offense, such as the time it occurred, what was wrong, and any prior warnings that were issued concerning the incident, in the second paragraph.

A clear account of the verbal warnings given. The conduct they need to change. Consequences if the persons behavior doesnt change.
On every write up form, include: The employees full name and employee number. Time and date of the write up and of specific incidents. Reason for the write up, in detail. Witness accounts confirming the employees misconduct. Times and dates of previous write ups and incidents.
The letter should include clear details about what the behavior was, how it violated company policies, and any consequences that may result from future occurrences. Additionally, it should provide guidance on how the employee can improve their behavior moving forward.
How to write a disciplinary letter Address and date the letter. State the reason for the letter. Include details of the performance issue or misconduct. Detail any policies or other rules the employee has broken. Stipulate what changes you want to see. Explain the next steps in the disciplinary procedure.
Example of How to Write up an Employee Description of the Issue: On [date], at approximately [time], I observed that you arrived to work 20 minutes late. Corrective Action: This written warning serves as a final notice regarding your tardiness. Improvement Plan: Consequences of Not Improving:
