Symptoms. Direct CCF often occurs days or weeks after a closed head injury. Patients present with the classic triad of chemosis (red eye syndrome), pulsatile exophthalmos (abnormal protrusion of the eyeball), and ocular bruit (blood flow sounds coming from the eye).
Diagnosis. A complete history and a detailed clinical exam with appropriate diagnostic testing can help identify and classify CCFs. In office modalities that can be performed include auscultation for an ocular bruit and pneumotonometry to assess for increased ocular pulse amplitude.
Signs and symptoms Onset is usually sudden. Direct carotid-cavernous fistulas are characterized by the triad of pulsatile proptosis, chemosis, and intracranial whistling. There is severe conjunctival congestion, hemorrhagic chemosis, ptosis, and pulsatile proptosis accompanied by a whistling.
Further classification is based on their etiologic and hemodynamic qualities. Direct CCFs are characterized by a direct connection between the internal carotid artery (ICA) and the cavernous sinus, whereas indirect CCFs result from an indirect connection involving cavernous arterial branches and the cavernous sinus.
Amongst the available imaging methods (Doppler ultrasonography, computed tomography, magnetic resonance imaging and digital subtraction angiography), angiography is considered the gold standard for the diagnosis and classification of cavernous sinus arteriovenous fistulae.
Carotid cavernous fistulas (CCFs) are a rare but potentially devastating cause of orbital symptoms, visual loss, and periocular disfigurement. Carotid cavernous fistula patients typically present with proptosis, elevated intraocular pressure, prominent tortuous conjunctival vessels, and sometimes headache.
Type A, or direct carotid-cavernous fistula, are the most frequent type of CCF. Trauma is the most common cause of direct CCFs and accounts for the higher prevalence in young men. Traumatic CCF are usually unilateral, but can occur bilaterally in 2% of patients.
Indirect CCFs occur spontaneously and can be caused by a docHub tissue disorder. Although less common, they can also be caused by ruptured aneurysms or atheroscl (hardened) arteries. Indirect CCFs occur more often in postmenopausal women who have high blood pressure.
