Direct CCF are high-flow fistulas occurring due to a tear in the carotid artery most commonly from either penetrating or non-penetrating head trauma [1]. Direct CCF also occur secondary to cavernous aneurysm rupture and from iatrogenic trauma following oromaxillofacial and neurosurgical procedures [2,3].
CCFs can be classified based on the hemodynamic properties, the etiology, or the anatomy of the shunt. Hemodynamically, the fistulas can be classified as: Low flow fistulas, and. High flow fistulas.
Diagnosis of Carotid-Cavernous Sinus Fistulas Your neurologist may order imaging scans to get a look at your eye, nearby blood vessels and cavernous sinus area: Ultrasound. Computerized tomography (CT) scan. Magnetic resonance imaging (MRI)

A carotid-cavernous fistula (CCF) is an arteriovenous fistula with an abnormal connection between the carotid artery and cavernous sinus that can be sight and life-threatening.
It can severely limit the activities you're able to do and is often eventually fatal. But it's very difficult to tell how the condition will progress on an individual basis. It's very unpredictable. Lots of people remain stable for many years, while in some cases it may get worse quickly.
A carotid-cavernous fistula (CCF) is the result of an abnormal vascular connection between the internal carotid artery (ICA) or external carotid artery (ECA) and the venous channels of the cavernous sinus. CCFs are classified based on the arterial system involved, hemodynamics, and etiology.
Overall, endovascular intervention offers a 90–100% cure rate with a low rate of complications35, 45, 47, 48, 49 and an acceptably low mortality rate of <1%. Minor transient complications, including haematoma, facial pain, and ocular motor nerve palsies, occur in 1–30% of cases.
The first treatment option should be endovascular embolization with a combination of detachable balloons, coils, stents, or liquid embolic agents. The procedure can be performed from either an arterial or venous approach. Use of these materials and techniques can yield a high cure rate with minimal complications.
