There are two types of carotid-cavernous sinus fistulas. Direct CCFs usually result from a torn carotid artery. This type is often caused by head trauma; between 70 and 90 percent of direct CCFs are caused by injury to the base of the skull (basal skull fractures).
Direct CCF are high-flow fistulas occurring due to a tear in the carotid artery most commonly from either penetrating or non-penetrating head trauma [1].
Introduction. Carotid-cavernous fistula (CCF) is an abnormal connection between the carotid artery and/or its branches and a large vein called the cavernous sinus. The cavernous sinus is located behind the eye and receives blood from brain, orbit, and pituitary gland.
Direct CCF are high-flow fistulas occurring due to a tear in the carotid artery most commonly from either penetrating or non-penetrating head trauma [1]. Direct CCF also occur secondary to cavernous aneurysm rupture and from iatrogenic trauma following oromaxillofacial and neurosurgical procedures [2,3].
CCFs can be classified based on the hemodynamic properties, the etiology, or the anatomy of the shunt. Hemodynamically, the fistulas can be classified as: Low flow fistulas, and. High flow fistulas.
A carotid-cavernous fistula (CCF) is the result of an abnormal vascular connection between the internal carotid artery (ICA) or external carotid artery (ECA) and the venous channels of the cavernous sinus. CCFs are classified based on the arterial system involved, hemodynamics, and etiology.
Most CCFs are not life threatening, but the involved eye is at risk. Main indications for treatment include glaucoma, diplopia, intolerable bruit or headache, and severe proptosis causing exposure keratopathy.
It can severely limit the activities you're able to do and is often eventually fatal. But it's very difficult to tell how the condition will progress on an individual basis. It's very unpredictable. Lots of people remain stable for many years, while in some cases it may get worse quickly.
Direct CCFs often present with a classic triad of pulsatile exophthalmos, orbital bruit, and chemosis. Patients may report diplopia, ocular redness, orbital/retro-orbital pain, swelling, swishing or buzzing sounds, headache, or vision loss.
CCFs can be classified based on the hemodynamic properties, the etiology, or the anatomy of the shunt. Hemodynamically, the fistulas can be classified as: Low flow fistulas, and. High flow fistulas.
