A doctor can usually diagnose a torn ACL from a physical exam, although magnetic resonance imaging (MRI) is helpful. Getting an MRI is also important to find out if other parts of the knee have been injured.

Primary signs of ACL rupture are increased signal on MRI, fiber discontinuity, abnormal orientation and undetectable fibers [11]. MRI-derived measures of the signal intensity (SI) of the ACL graft have been described as an independent predictor of graft properties [9].
Symptoms of an ACL Tear: Rapid swelling. A loud popping in the knee. Knee instability where the knee feels like it will buckle and cannot support the weight. Loss of range of motion.
PCL and ACL Tears A ligament disruption (also known as a ligament tear) may be partial or complete, causing pain, swelling, and loss of function. Ligament tears are graded by their severity: Grade 1 tear: Microtear and stretching with ligament integrity intact. Grade 2 tear: A partial tear with some joint looseness.
The authors found that fear of re-injury, motivation and psychological readiness are just as important as physical factors for optimal ACL recovery. In fact, several studies have found that fear of re-injury is the most common reason for patients not returning to sport.
