Certified copies of the death certificate can be requested from the New Mexico Department of Health Bureau of Vital Records and Health Statistics: 505-827-0121 or visit the New Mexico Department of Health website.
(1) To amend a birth certificate, application may be made by both parents, the legal guardian or court ordered custodian, the registrant if 18 years of age or over, a legal representative for the registrant or parents, or the individual responsible for filing the original certificate.
To officially change your name, you first need to file your marriage certificate in our office. Then, you need to purchase certified copies to be taken to places like the Social Security Administration, the Motor Vehicle Department, and other places that require proof of marriage.
Copies of marriage certificates are available at the Civil Register Office where the ceremony was performed.
Copies of marriage licenses are available from the county clerk of the county where the marriage license was issued. Copies of divorce decrees are available from the district court where the court order was filed.
Requirements Both applicants need to be present. Must be at least 18 years of age. Parental consent if 16 or 17 years of age. Current legal picture ID or birth certificate. Proof of Social Security number (SS card, W2, or Tax Return) Fee is $25.00 cash or credit card, cash only after 5:00 pm.
New Mexico does not provide a statewide public repository for marriage records. Instead, marriage records are maintained by the County clerk in the county where the license was issued. Residents with tangible interest can obtain copies of such records by applying to the county office.
Marriages may only be performed in the State of New Mexico. The signed license must be returned to the County Clerks Office within 90 days of the ceremony. Must complete a Marriage Application Information Form in the County Clerks Office. Must pay a $25 fee at the time of application.
