A carotid-cavernous sinus fistula (CCF) is an abnormal communication between arteries and veins within the cavernous sinus. CCFs may be classified into four types: direct fistulas (Barrow type A) and dural, or indirect, fistulas (Barrow types B, C, and D).
Type A (high flow) CCFs defined as, when there was a direct communication between the internal carotid artery cavernous segment and the cavernous sinus. Type B (low flow) CCFs defined as, when there is an abnormal communication between the cavernous sinus and meningeal branches of the internal carotid artery.
Introduction. Carotid-cavernous fistula (CCF) is an abnormal connection between the carotid artery and/or its branches and a large vein called the cavernous sinus. The cavernous sinus is located behind the eye and receives blood from brain, orbit, and pituitary gland.
CCFs can be classified based on the hemodynamic properties, the etiology, or the anatomy of the shunt. Hemodynamically, the fistulas can be classified as: Low flow fistulas, and. High flow fistulas.
Type A are direct fistulas, whereas types B, C, and D are indirect fistulas. CCFs are further classified into traumatic or spontaneous etiology and high or low flow based on hemodynamic properties (Table 2).

Ventilation-perfusion (V/Q) mismatch occurs when either the ventilation (airflow) or perfusion (blood flow) in the lungs is impaired, preventing the lungs from optimally delivering oxygen to the blood.
The most common findings include arterialization of the conjunctival veins, chemosis, proptosis, diplopia, bruit, retro-orbital headache, elevated intraocular pressure, and a decrease in vision. The classic triad of ocular symptoms seen in direct CCFs are less commonly seen in indirect CCFs.
