Write-ups for employees are formal warnings that document ongoing performance or behavioral issues and offer improvement plans. Supervisors typically issue a write-up if incidents continue to occur even after the employee receives a verbal warning.
What to Include in a Disciplinary Form The employees name and the date of the write-up. Clearly state why they are being written up. How many times this employee has been written up. Clearly state details about the problem. Give the employee a deadline to fix the problem. Always have them sign and date the write-up.
Work write-ups happen when an employee breaks a policy or repeatedly does something they shouldnt. Examples include misusing company software, showing up late to work regularly, ignoring safety procedures and performing below expectations.
What should a write-up form contain? Employee name, position, and ID number. Type of warning. Offense committed. Description of the incident. Improvement plan. Consequences of a repeat offense. Area for manager and employee to sign and date.
Follow these eight easy steps to make sure you get it right and define your write-up format. Dont do it when youre angry. Document the problem. Use company policies to back you up. Include any relevant witness statements. Set expectations for improvement. Deliver the news in person (and proof of receipt)
Make sure each employee write-up form includes: The employees name or ID Number. The employees position. A specific recounting of the offense committed. The type of warning or discipline enacted. A plan for improvement. Any and all managers or decision-makers who should be aware of the situation.
