You must submit the Form MCS-150 and select the reason for filing as Out of Business. Please provide a copy (as an attachment) of the Drivers License from the individual who signed the certification statement either #31 on the Form MCS-150, #37 on the Form MCS-150B, or #22 on the Form MCS-150 C.
There are several ways to update your company information with the FMCSA (there is no charge for USDOT number updates): Online: You can update your USDOT number company information on the Licensing and Insurance (LI) web site by filing an MCS-150 (Motor Carrier Identification Report).
Failure to complete a Biennial Update will result in deactivation of your USDOT number and may result in civil penalties of up to $1,000 per day, not to exceed $10,000. Updating your information is free.
How do I change the company name on my USDOT registration? Submit a ticket through our website at ask.fmcsa.dot.gov (scroll to the bottom of the page, complete the required fields, and upload your documents). Fax the form to 202-366-3477 or 606-330-3802. Mail the form using the instructions provided on the form.
If you do not need your USDOT Number or MC Number any longer you can request to cancel them both out. The process will take at least 2-3 weeks in order to cancel out your USDOT Number. USDOT Numbers that are not deactivated are still subject to federal mandates and you may be penalized for noncompliances.
