A Pap smear is a test to check for cervical cancer. If you have a cervix and are between the ages of 21 and 65, you should have the test every 3 years. The test is performed by inserting a speculum into your vagina and collecting some cells from your cervix that are analyzed at a lab.
Higher risk: Your test results show either HPV infection with types 16 and/or 18, high grade cell changes on your cervix, or persistent infection with one of the other high-risk HPV types (not 16/18). It is important that you have a further follow-up because you may be at a high risk of developing cervical cancer.
Low-grade cervical changes are unlikely to become cervical cancer. If the changes are moderate- to high-grade, your doctor will evaluate more options. These changes create a higher risk of cervical cancer. If there are moderate to severe changes in your cervix, your doctor may want to remove the cervical tissue.
High-grade squamous intraepithelial lesions (HSIL): There are moderately or severely abnormal cervical cells that could become cancer in the future if not treated. Your health care provider will likely ask you to come back for a colposcopy.
An abnormal Pap test result rarely means cancer is present, especially in someone who has regular, annual Pap tests. An abnormal result means there are cells in the cervix that look unusual, which can be caused by yeast or bacterial infection, irritation, benign growths, hormonal changes, or pre-cancer.
Most of the abnormal cells found during a Pap test are the result of a cervical or vaginal infection and are not cancerous. Abnormal Pap tests are very common. In fact, of the 3 million women with abnormal Pap tests each year, less than 1% (13,240 cases) will be diagnosed with cervical cancer.
A normal, also called negative, Pap smear result indicates that no evidence of abnormal cells were found in the sample. An abnormal, or positive, result on a Pap smear indicates that abnormal cells were detected in the sample and additional treatment or testing may be necessary.
HPV is what usually causes an abnormal Pap smear result, says Ferris. However, those within the 21-29 age bracket are not tested for HPV unless their Pap finds atypical cells that may indicate HPV. HPV can get into cells, causing them to change, continued Ferris.
