A Control Number (box D) is used by many payroll departments to uniquely identify a W-2 in their system. If your W-2 doesnt have one, its no big deal. You can simply leave it blank.
Arrange any supporting statements in the same order as the schedules or forms they support and attach them last. Attach a copy of Forms W-2, W-2G and 2439 to the front of Form 1040. Also attach Forms 1099-R if tax was withheld. Use the coded envelope included with your tax package to mail your return.
Generally, the amount in Box 14 is for informational purposes only; however, some employers use Box 14 to report amounts that should be entered elsewhere on your return. Note. Employers use many different abbreviations and codes in Box 14 to report a variety of information.
The IRS accepts returns that are stapled or paperclipped together. However, any check or payment voucher, as well as accompanying Form 1040-V, must not be stapled or paperclipped with the rest of the return, since payments are processed separately.
