There are two types of cerclage: Transvaginal cerclage. Ob-gyns most often do a cerclage by docHubing through the vagina to place stitches in the cervix. Cerclage usually is done in a hospital.
Glossary Cerclage. The most commonly utilized cerclage is the McDonald cerclage shown at right. Two other types of cerclage are the Shirodkar cerclage which is placed underneath the skin of the cervix, and the abdominal cerclage which is placed around the lower part of the uterus through an abdominal incision.
There are two main types of stitch: The Shirodkar stitch is placed under the surface of your cervix near where it meets the womb. The MacDonald stitch is placed lower down on your cervix, nearer to your vagina. The stitches are inserted vaginally and usually at around 13 or 14 weeks of pregnancy.
Before pregnancy, your cervix the lower part of the uterus that opens to the vagina is normally closed and firm. As pregnancy progresses and you prepare to give birth, the cervix gradually softens, decreases in length (effaces) and opens (dilates).
The McDonald and Shirodkar techniques, or modifications thereof, are the most commonly used methods for placing a cerclage.
Cerclage is usually done transvaginally as either a McDonald or a Shiradkor procedure. When these 2 procedures are unsuccessful or difficult to perform, the transabdominal cerclage procedure is done.
Transabdominal cerclage (TAC) is a highly effective surgical treatment for incompetent cervix. With this condition, the cervix is too weak to carry a full-term pregnancy, often leading to miscarriage in the second trimester.
Elective cervical cerclage, by whichever technique employed, carries risks for the pregnancy. Surgical manipulation of the cervix can cause uterine contractions, bleeding or infection which may lead to miscarriage or pre‐term labour.
