Suture material Nylon: Nylon creates a type of natural monofilament suture. Polypropylene (Prolene): This material creates a monofilament suture. Silk: Silk sutures are typically braided and made naturally.
Then there are absorbable tacks, which are pricier alternatives that can be fully absorbed by the body within 12 to 18 months, depending on the manufacturer. Like suture, a misplaced tack may result in an injury to the anatomy a nerve or vessel, say that could necessitate reoperation.
The mesh needs to remain in place until tissue integration is complete. Complete integration usually happens around 2-3 weeks after surgery [10]. In addition to tissue integration, chronic pain after the surgery is also important to consider and a larger problem compared to hernia reoccurrence [11].
In general, surgeons typically use either polypropylene or polydioxanone sutures for fascia, depending on how strong the repair needs to be. Polypropylene is also very common in cardiovascular surgery. Deep dermis closure is with either polyglycolic acid or poliglecaprone 25 sutures.
The tack is constructed of an absorbable synthetic polyester copolymer derived from lactic and glycolic acid. AbsorbaTack is offered in a long version for laparoscopic techniques in both 15 and 30 tack configurations, as well as a short version for open hernia repair in a 20 tack configuration.
Tackers (mesh fixation devices) are often used for securing the mesh in the laparoscopic repair of ventral, inguinal and hiatus hernia. By and large, their use is safe but occasionally the tackers are the cause of postoperative complications.
Although all these methods differ in their approach to reconstructing the abdominal wall, they are all similar in that they use the bodys natural tissue instead of mesh, and use permanent sutures to approximate the surrounding tissue around the hernia defect and repair the posterior wall of the inguinal canal.
Application is during hernia repair and tacks will essentially absorb within the body after 1 year.
Conclusion: A non-absorbable or a late absorbable suture is recommended for open non-mesh groin hernia repair.
