Each hospital assigns its own medical record number (MRN) to each patient and a separate encounter number for each visit.
How you make your request will depend on your providers processes. You may be able to request your record through your providers patient portal. It is a common practice that medical record numbers contain six digits. The six digits are then further subdivided into three parts by the use of a hyphen, thus making it easier to read. For example, rather than reading 123451; you would read 12-34-51.
The patients legal name, date of birth, gender, Social Security number, address, telephone number, guarantor, subscriber, or next-of-kin are key identifying elements that assist in establishing the proper individual.
A UPI is a single medical identification number, much like a Social Security number (SSN) or some other identification (such as biometrics or smartphones), that is unique to each person and used to organize their medical information.
