Employers. The Affordable Care Act requires employers to report the cost of coverage under an employer-sponsored group health plan on an employees Form W-2, Wage and Tax Statement, in Box 12, using Code DD.
DD Code DD includes cost of employer-sponsored health coverage. Information only. EE Designated Roth contributions under a governmental 457(b) plan. This amount doesnt apply to contributions under a tax-exempt organization Section 457(b) plan.
Multiply total cost of health care per month by the number of months the employee was enrolled in the plans for that tax year. Report the total in box 12 on the W-2 form using code DD.
The W-2 box 12 codes are: A Uncollected Social Security tax or Railroad Retirement Tax Act (RRTA) tax on tips. Include this tax on Form 1040 Schedule 2, line 13. B Uncollected Medicare tax on tips. Include this tax on Form 1040 Schedule 2, line 13.
Items entered in box 14 must be labelled with a code that provides a brief description of the item. There are no standardized codes for box-14 because the federal government doesnt provide standardized codes for box 14 items, so employers can assign their own code that adequately describes the reported item.
Look at the Income section on the front page of the 1040 federal return for 2023, and then go to Line 1h to report strike pay or lockout benefits under Other earned income.
Box 12 codes A: Uncollected social security or RRTA tax on tips reported to your employer. AA: Designated Roth contributions under a section 401(k) plan. B: Uncollected Medicare tax on tips reported to your employer (but not Additional Medicare Tax) BB: Designated Roth contributions under a section 403(b) plan.
