The anterior cruciate ligament (ACL) is one of the ligaments inside your knee – it joins your thigh bone (femur) to the front of your shin bone (tibia). An ACL injury is a partial or complete tear, a stretched ligament, or a detachment of the ligament from your bone.
Grade 3. Grade 3 ACL tears happen when the ACL is torn completely in half and is no longer providing any stability to the knee joint.
The ACL tears more often than any other ligament—there may be as many as 200,000 ACL injuries per year in the United States—because of the lack of muscle support for twisting or rotational movements around the joint.
Signs and symptoms of an ACL injury usually include: A loud pop or a "popping" sensation in the knee. Severe pain and inability to continue activity. Rapid swelling. Loss of range of motion. A feeling of instability or "giving way" with weight bearing.
Grade 2 tears are rare. In this case, the ACL is stretched and partially torn. You notice some instability in the knee joint so that it collapses periodically when you stand or walk.
Imaging evaluation of ACL tear often begins with knee magnetic resonance imaging (MRI). Studies have shown MRI to have an overall accuracy of 85.0%, a sensitivity of 82.5%, and a specificity of 92.8% for diagnosing ACL tears.
The Lachman test is the most accurate test for detecting an ACL tear. Magnetic resonance imaging is the primary study used to diagnose ACL injury in the United States. It can also identify concomitant meniscal injury, collateral ligament tear, and bone contusions.
ACL Classification Injuries are graded based on severity: Grade 1, Grade 2, and Grade 3. Grade 1 injuries are the least severe, and Grade 3 injuries are the most severe. All three grades are varying levels of a sprain of the ACL. Sprains refer to ligament injuries, whereas strains refer to muscle injuries.
Types of ACL Injuries Grade 1 — least severe ACL injury. Means you stretched but didn't quite tear, the ACL. The ligament can still keep the knee joint stable. Grade 2 — a partial tear. Means you stretched the ACL, making it loose. ... Grade 3 — most severe ACL injury. Means a complete or near complete tear.
Types of ACL Surgery Autograft. Your doctor uses a tendon from somewhere else in your body (like your other knee, hamstring, or thigh). Allograft. This type of graft uses tissue from someone else (a deceased donor). Synthetic graft. This is when artificial materials replace the tendon.
