A chosen first name is a name that you choose to be called. It is different from your name assigned to you at birth.
Try just saying professor, but without the surname, so it is descriptive rather than a title. Or if you really want to make the point, you could use sir or madam.
A preferred/chosen name is any name a student chooses to use other than their legal name.
A first name by which an individual wishes to be identified that is other than the individuals legal name is a preferred first name. This is analogous to a nickname. Students and employees can submit a preferred first name online.
