The problem in heart failure is that the heart isnt pumping out enough blood each time it beats. This is called low stroke volume. To maintain your cardiac output, your heart can try to beat faster (increase your heart rate) or pump more blood with each beat (increase your stroke volume).

The hallmark of heart failure is dyspnea. The classic combination of raised jugular venous pressure (JVP), peripheral edema, palpable liver, basal crepitations, tachycardia, and a third heart sound is well known.
As the force of contraction increases, the heart is able to push more blood out of the heart, and thus increases the stroke volume. The final determinant of stroke volume is afterload. Afterload represents all the factors that contribute to total tension during isotonic contraction.
Left-sided heart failure symptoms include: Awakening at night with shortness of breath. Shortness of breath during exercise or when lying flat. Chronic coughing or wheezing.
Following this initial decline in pumping capacity of the heart, a variety of compensatory mechanisms are activated, including the adrenergic nervous system, the renin angiotensin system, and the cytokine system.
Systolic Heart Failure Symptoms The hallmark feature of systolic heart failure is shortness of breath, also called dyspnea. A person with early systolic heart failure might get short of breath only when exercising.
