STANDARDIZED MORTALITY RATIO (abbreviated SMR) is the number of observed deaths in the study population divided by the number of expected deaths (calculated from indirect adjustment) and multiplied by 100 (Lilienfeld Stolley, 1994; Last, 2001).
The SMR may be quoted as either a ratio or a percentage. If the SMR is quoted as a ratio and is equal to 1.0, then this means the number of observed deaths equals that of expected cases. If higher than 1.0, then there is a higher number of deaths than is expected.
A ratio greater than 1.0 indicates that more mortality has occurred than would have been expected, while a ratio less than 1.0 indicates that less mortality has occurred. The decimal fraction shows the percentage comparison.
Direct and indirect methods of standardization address confounding when comparing the rate of a given outcome (like mortality) between two populations that differ for important prognostic factors (like age, gender and social-economical status). These two methods are used for both descriptive and analytical purposes.
The SMR (or SIR) is the ratio of observed number of deaths (or incident cases) in the cohort to the number of deaths (or incident cases) that would be expected, for example, on the basis of age- and gender-specific death (or incidence) rates in the general population.
Direct standardization is used when the number of events or the mortality rates in each age group within the population, is known, whereas indirect standardization is used when the number of events or the mortality rates in each age group within the population is not known.
The age-specific fertility rate is calculated as the quotient of the numerator divided by the denominator for each age group, multiplied by 1000. The result is an average rate over the 36-month period, expressed as an annual rate per 1000 women.
Direct method The adjusted or standardized rate is obtained by dividing the total of expected cases by the standard population.
The standardized mortality rate (SMR) is the ratio of the number of deaths observed in a population over a given period to the number that would be expected over the same period if the study population had the same age-specific rates as the standard population.
