this is a video of ACL reconstruction graft preparation here I am on the back table whip stitching with an Rx fiber loop an allograft I typically do not use a graft holder I use a number two fiber loop on either end and whip stitch till the end and then cut the needle and I do this to both ends Here I am pulling full tension I am now going to run it through a sizer to get the diameter of the graft so I know what diameter tunnel to drill once I have sized it I then put tension on it well the autoscopic portion of the case is being done Here I am with a marker I am marking the tightrope the appropriate length so I know when the button is expected to flip on the lateral femoral cortex and I also mark the appropriate graft length so I know when to cease pulling tension on the tensioning sutures

Tunnels are created by drilling through the shin bone and thigh bone. The graft is passed through these tunnels and secured with screws to replace the original ACL. The incisions are then closed with sutures and covered with dressings. The procedure takes about 1-1/2 to 2 hours.
In ACL reconstruction, a replacement ligament (graft) is attached to tunnels drilled into the end of the femur (thigh bone) and tibia (shin bone). Often screws are used to attach the graft to the bone. Traditionally, metal screws have been used.
The strongest option is the BTB graft. The graft incorporates more solid into the bone due to the bone plugs on either end of the tendon. However, BTB grafts have been known to have the slowest recovery time when it comes to meeting rehab milestones and returning to sport.
Your surgeon will make a tunnel in your bone to pass the new tissue through. The graft tissue will be positioned in the same place as the old ACL and held in place with screws or staples that will remain in your knee permanently.
These plugs help to anchor the graft that will become the new ACL. The surgeon inserts the new ACL into the femur and tibia using a flexible guide wire. Screws are used to secure the plugs of bone. Over time, these plugs will grow into the surrounding bone.
Description of the intervention Arthroscopically assisted ACL reconstruction is typically performed through two standard portals (small stab incisions through which a camera (arthroscope) and instruments are introduced). The general aim is to place the graft as near as possible to that of the native ligament.
Hamstring tendon grafts are one of the most commonly used grafts for ACL reconstruction. The semitendinosus tendon with or without gracilis tendon is harvested, from ipsilateral leg. They are used as quadruple stranded grafts and are comparable to native ACL.
The signs of ACL graft failure can include swelling, pain within the knee, locking within the knee, a mechanical block (which can be due to a bucket-handle tear of the meniscus), lack of full motion, and difficulty with twisting, turning, and pivoting.
Your surgeon will drill sockets or tunnels into your thighbone and shinbone to accurately position the graft, which is then secured to your bones with screws or other devices. The graft will serve as scaffolding on which new ligament tissue can grow.
ACL surgery is usually done by making small incisions in the knee and inserting small tools for surgery through these incisions (arthroscopic surgery). In some cases, it is done by cutting a large incision in the knee (open surgery).
