The physical examination remains a mainstay of orthopaedic diagnosis, and multiple physical examination maneuvers can be used to evaluate a possible anterior cruciate ligament (ACL) injury. Historically, the most commonly used tests are the Lachman test, the anterior drawer test, and the pivot shift test.
Symptoms A loud pop or a popping sensation in the knee. Severe pain and inability to continue activity. Rapid swelling. Loss of range of motion. A feeling of instability or giving way with weight bearing.
An MRI can show the extent of an ACL injury and signs of damage to other tissues in the knee, including the cartilage. Ultrasound. Using sound waves to visualize internal structures, ultrasound may be used to check for injuries in the ligaments, tendons and muscles of the knee.
Four tests are commonly used to diagnose or determine ACL injury: the Lachman test, the anterior drawer test, the pivot shift test, and the lever sign test.

The gold standard in diagnosis of anterior cruciate ligament (ACL) injury is diagnostic arthroscopy.
For example, a Lachman test can help determine if there is an ACL tear, while a varus/valgus test can detect if the MCL is injured. Its important to note that even with a detailed physical examination and special tests, an accurate diagnosis can only be made by an experienced orthopedic doctor.
Secondary signs include bone marrow edema (secondary to bone contusion) in more than half of ACL tears. The characteristic ACL bone bruising involves the middle third of the Lateral femoral condyle and the posterior third of the lateral tibial plateau.
