Fast facts. The anterior drawer test is a physical examination doctors use to test the stability of the knees anterior cruciate ligament (ACL). Doctors may use this test, along with images and other exams, to determine if a person has injured their ACL and recommend treatment options.
The short answer is yes. After the pain and swelling subsides and if there is no other injury to your knee, you may be able to walk in straight lines, go up and down stairs and even potentially jog in a straight line.
On pulling the tibia anteriorly, an intact ACL should prevent forward translational movement of the tibia on the femur (firm end-feel). Anterior translation of the tibia associated with a soft or a mushy end-feel indicates a positive test.
Positive: absence of an end-feel and the proximal tibia falls back (excessive translation). The amount of posterior movement determines the grade of PCL tear.
Signs and symptoms of an ACL injury usually include: A loud popping in the knee. Severe pain. Rapid swelling. Loss of range of motion. Knee instability where the knee feels like it will buckle and cannot support the weight.
Many people hear a pop or feel a popping sensation in the knee when an ACL injury occurs. Your knee may swell, feel unstable and become too painful to bear weight.
The Lachman test is a specific clinical exam technique used to evaluate patients with a suspected anterior cruciate ligament (ACL) injury. The test relies on proper positioning and technique and is regarded as the most sensitive and specific test for diagnosing acute ACL injuries.
If the tibia, or shinbone, has more movement, or if the ligament is loose compared with the other knee, the anterior drawer test is considered to be positive.
The test is considered positive if there is a lack of end feel or excessive anterior translation relative to the contralateral side. Theoretically, the anterior translation if less than 6mm. If it is more than 6mm, the test is considered positive.
If the tibia pulls forward or backward more than normal, the test is considered positive. Excessive displacement of the tibia anteriorly suggests that the anterior cruciate ligament is injured, whereas excessive posterior displacement of the tibia may indicate injury of the posterior cruciate ligament.
