Signs and symptoms of an ACL injury usually include: A loud pop or a popping sensation in the knee. Severe pain and inability to continue activity. Rapid swelling.
It can be easy to confuse the two injuries, as both a torn ACL and a torn MCL exhibits similar symptoms, including: swelling, inflammation, extreme pain and possible bruising. The main difference between an ACL tear and an MCL tear is that an ACL tear will have a distinctive popping sound, while an MCL tear will not.
The most common provocative maneuvers used for assessing the ACL are the Lachman, anterior drawer, and pivot shift tests. With all three testing maneuvers, it is extremely important to compare the injured knee to the contralateral side. The Lachman test is considered the most reliable test for assessing an ACL injury.

Magnetic resonance imaging (MRI) In experienced assessors, the combination of patient history and clinical examination will often be sufficient to diagnose an ACL rupture. However, pain and effusion in an acute setting may make it difficult to detect the injury during clinical examination.
The anterior Lachman test, anterior drawer test and the pivot shift test, which are summarised in Table 1, are the most commonly known physical tests used to assess the integrity of the ACL (Benjaminse 2006; Leblanc 2015; Malanga 2003; Scholten 2003; Solomon 2001).
