You can send a letter of intent when you have finished all or most of your medical school interviews, including your interview at your #1 choice of school. You can also send a letter of intent when you have been waitlisted at your top school after completing your interview.

A letter of interest expresses your interest and excitement about the schools academic programs, offerings, and environment without making a commitment to that school. In a letter of interest, you should define how you would fit within the schools program and what you would bring to it.
How to write a healthcare recommendation letter Gather helpful information from the applicant. Research the organization or school. List your contact information and use a formal greeting. Allow plenty of turnaround time. You want to be sure the letter writer has the opportunity to write a thoughtful, complete letter without worrying about an unrealistic deadline. Our Cracking Med School Admissions team advises students to wait until a decision comes, and then to write a Letter of Intent. The one exception to this is if during your interview day, the admissions officers specifically say they encourage students to write continued interest and update letters.
When you could send it: One month after your interview, whether youre placed on the waitlist or havent yet heard back. You may send a second letter of intent if over two months have passed since your initial one and you have meaningful updates to share.
Unlike Letters of Interest, Letters of Intent are intended to be sent after an interview, informing a program of your intent to rank them highly. These should be sent approximately 2 weeks before the Rank Order List opens to 2 weeks before the Rank Order List Deadline to the top 3-7 programs on your Rank Order List.
The list of key parts are: Introduction and statement of recommendation. List of specific reasons you are recommending them to the position. Personal story with evidence of their qualities (soft and hard skills) Closing statement with contact information. Signature.
