What does personalised care and support planning mean for patients and carers? provides an introduction to care and support planning, introduces the 4 steps of the approach and sets out what should happen at each step: prepare, discuss, document, and review.
The registered nurse is responsible and accountable for assessing educational needs in respect to advance directives and to insure that the clients and staff members have the sufficient knowledge to make sound and knowledgeable decisions relating to these important aspects of client care.
An easy way to start the conversation is to ask if they have any documents already prepared, if they have talked to anyone else about advance directives, and if they would be comfortable sharing how they feel. You can also use resources like the Serious Illness Conversation Guide (see Resources).
Advance directives do not need to be docHubd, only witnessed, signed and dated. Two witnesses are required. Only one of them may be a family member or caregiver. The lack of advance directives will not impact your access to care.
The two most common advance directives for health care are the living will and the durable power of attorney for health care. Living will: A living will is a legal document that tells doctors how you want to be treated if you cannot make your own decisions about emergency treatment.
The clinician then starts the meeting by determining what the patient knows about advance care planning and assists in choosing a surrogate, who is included in the discussion. Both the patient and surrogate are asked what they know about the illness, including prognosis.
In an ACP discussion, patients are guided to understand, reflect upon, and discuss their goals, values and beliefs; then led to indicate their preferences with regard to future healthcare treatments. ACP also involves nominating a substitute decision-maker who can make decisions on the patients behalf.
