Box 14 Employers can use W-2 box 14 to report information like: A member of the clergys parsonage allowance and utilities. Any charitable contribution made through payroll deductions. The Notice states that elective deferral contribution amounts to a Roth SEP or Roth SIMPLE IRA are reported on a Form W-2 in Box 12 using code S (for SIMPLE) or code F (for SARSEP) and are included in the amounts shown in Boxes 1, 3, and 5.
Form W-2, box 12, code V represents income from the exercise of non-statutory stock options. Since the amount is already reported in box 1 (wages, tips), which flows to Form 1040, line 1 (line 7 in Drake17 and prior), the amount in box 12 does not flow to any another section or supporting form.
