How to Write a DNR Form Write the name of the patient and then state where you or that person lives. Make sure to make the requests known, that CPR, or another treatment, is to be withheld. Acknowledgement: The people who are in charge of the patient must sign the document.
For example, someone with terminal cancer might write that they do not want to be put on a respirator (breathing machine) if they stop breathing. This action can increase their control over their death and their peace of mind, and eventually reduce their suffering.
In summary, the minimum requirement in order for a DNR form to be valid is the valid signature and date of the patient, or of their legally recognized healthcare agent in the event that they are incapacitated or otherwise unable to express their wishes.
The only instance in which a family member can revoke a DNR is when that same family member is also the patients healthcare agent. If the patient authorized that family member to serve as their agent through their Medical Power of Attorney document, then that family member does have the power to cancel the DNR.
While each state has its own unique paperwork that is appropriate for citizens of that state, there are two general types of DNR orders that are most common across the country: do not resuscitate in hospital and do not resuscitate out of hospital.
Documentation in the patients medical record of a decision not to do CPR must include both a Physicians Order and an entry in the Progress Notes. Use of the standardized DNR Order Form is preferred, but is not required in cases where the attending physician prefers a narrative method of documentation.
