you have legal rights in regard to requesting medical records generally what you need to do is go to your doctor request the medical records usually they have those their own file at the doctors office and they can get those far you make copies now what I normally tell my clients is to call a day or two ahead of time dont just show up and say I need my medical records call a day or two ahead of time give them an opportunity to get the medical records copied for you many times though if the medical records are are quite old lets say over a year or two old many Medical Care Providers now uh hire a a separate company to uh do like microfilm and digitize the medical records and they have to request these old medical records from that particular company but generally speaking you can get those medical records yourself if theyre your own medical records if theres someone elses medical records such as a loved one lets say uh you may have to have authorization to do that also you may ha

There is no legal requirement to not use blue ink on medical records. However, some healthcare organizations may have policies in place that require or prefer black ink be used instead. This is because the color of the ink can affect the readability of photocopies and scans of medical records.
Color coding refers to the use of color on folders to aid in the prevention of misfiling and in the location of misfiled records. Color bars in various positions around the edges of folders (known as blocking) create distinct patterns of color in various sections of the file.
All Medical Records, regardless of form or format, must be maintained in their entirety, and no document or entry may be deleted from the record, except in ance with the destruction policy (refer to section IX). Handwritten entries should be made with permanent black or blue ink, with medium point pens.
Most commonly, either blue or black ink is used for signing documents. While both are acceptable, many people consider blue the optimal choice. The reasoning behind this is that the color will stand out among the walls of black text on the document while still being dark enough to read.
Ideally, all entries in the medical record should be made in black ink. This would make it simple to produce a photoreproduction and ensure that the subsequent copies would be legible.
Most medical hand-written documentation is done in black ink. It is thought to be the most easily copied and fade resistant.
