If an employee has an amount reported in box 12 of their Form W-2 (Code W) for employer contributions to your health savings account (including contributions through a cafeteria plan), then the employee should complete IRS form 8889 Health Savings Account.
How to read your W-2 Box 1: Details how much you were paid in wages, tips, bonuses and other compensation. Box 2: Shows how much federal income tax was withheld from your pay by your employer. Box 3: Shows much of your pay in Box 1 was subject to Social Security tax.
Box 14 Employers can use W-2 box 14 to report information like: A member of the clergys parsonage allowance and utilities. Any charitable contribution made through payroll deductions. Educational assistance payments. Health insurance premium deductions. Nontaxable income. State disability insurance taxes withheld.
Form W-2, box 12, code V represents income from the exercise of non-statutory stock options. Since the amount is already reported in box 1 (wages, tips), which flows to Form 1040, line 1 (line 7 in Drake17 and prior), the amount in box 12 does not flow to any another section or supporting form.
Box 12 is a general catch-all for recording various types of income. Box 12 consists of four sections, 12a-12d. These are just references to organize the space and dont have any docHub meaning. W2 Box 12 uses codes A through HH to identify a variety of employee benefits that must be reported to the IRS.
