Form W-2. Employers must complete, file electronically or by mail with the Social Security Administration (SSA), and furnish to their employees Form W-2, Wage and Tax Statement showing the wages paid and taxes withheld for the year for each employee.
How to fill out Form W-2 W-2 Box A: Employees Social Security number. W-2 Box B: Employer Identification Number (EIN) W-2 Box C: Employers name, address, and ZIP code. W-2 Box D: W-2 Boxes E and F: Employees name, address, and ZIP code. W-2 Box 1: Wages, tips, other compensation. W-2 Box 2: Federal income tax withheld.
Preparing Form W-2 Type all entries using black ink and, if possible, in a 12-point Courier font. Entries are read by machine. Handwritten, script, or italicized fonts are discouraged. Entries made in other than black ink cant be read.
In the event you lose your Form W2, or require a duplicate copy you can download a copy from Cal Employee Connect under the W-2 tab. You also can request one from the State Controllers Office.
Whether you complete Forms W-2 on your own, use payroll software, or have a tax preparer, you should be semi-fluent in knowing how to fill out Form W-2. As a generalization, Form W-2 boxes show identification, taxable wages, taxes withheld, and benefits information. Boxes A-F are straightforward.
