On the basis of these findings, they developed a linear regression equation for predicting the native ACL length: ACL length (in millimeters) = 0.4606 × Patient height (in centimeters) − 41.29.
The choice of graft should also be individualized to the patient's risk factors for failure, anatomy, sport, level of competition, age, and graft used in previous ACL surgery for cases of revision reconstruction.
It can be concluded that to decrease the risk of having a failure in hamstring ACL reconstruction, a graft of at least 8.5-mm should be used.
Most studies have reported that the ACL is composed of two bundles: AM and PL. 1,2,3,6 In many studies, the average length of the ACL is approximately 37 mm–41 mm7,8,9 The length of the ACL varies depending on the knee position.
In ACL reconstruction, a replacement ligament (graft) is attached to tunnels drilled into the end of the femur (thigh bone) and tibia (shin bone). Often screws are used to attach the graft to the bone.
In ance with the study conducted by Araujo et al.,24 the length of the ACL was measured at the level where both attachment sites of the ACL were visible on the selected MRI. The measurement was taken from the midpoint of the tibial insertion site to the midpoint of the femoral insertion site of the ACL (Fig 1).
Typically, two of the hamstring tendons are obtained by using a small incision just below the knee. Two long lengths of tendon are obtained that allow the graft to be doubled over and so achieve a quadruple graft.
The ACL can be injured or torn in a number of different ways. The most common mechanism is that of a sudden pivoting or cutting maneuver during sporting activity, which is commonly seen in football, basketball and soccer. The ligament can also tear due to work injuries or automobile accidents.
Some mild, partial ACL tears can heal on their own, while other tears require surgery. The only way to know if surgery is the right choice for your ACL injury is to have Dr. Nickson evaluate your injury using advanced diagnostic techniques.
Most people who tear their ACL feel pain and a "pop" in their knee when the injury happens. Their knee usually gets swollen soon after the injury. After the swelling goes down, someone with an ACL tear usually can walk. But the knee may feel unstable and can "give way" and make the person stumble or fall.
