You can also identify the additional insured on your policy from the Description of Operations section. Look for the additional insured entity name (or the attached endorsement) in its description. Point your Certificate Holders to these locations in your certificate of insurance.
Producer: The name of the insurer, agent, or broker who handled the insurance purchase and issued the certificate to the insured.
Producer: The insurance agent or broker who issued the certificate. Insured: The name and address of the person(s) or business covered by the policy. Insurance coverages: The type of insurance typically general liability, professional liability, commercial auto, excess liability, or workers compensation.
For example, one field is labeled certificate holder. The certificate holder is the party the certificate of insurance is provided to. So if youre hiring someone, and you requested the COI, then youre the certificate holder.
Film producers need this protection for their liabilities to others arising out of their operations. This contract protects them for their liabilities for unintentional bodily injury and property damage. It provides coverage for claims of bodily injury and/or property damage arising out of the filming of a picture.
Carrier names: name of the company that holds your insurance policy. They are responsible for paying when you file a covered claim.
For the Insured, you will need to provide the full named insured and complete mailing address. In the lower right-hand area, you will need to enter the carrier name for any carriers represented as providing coverage on the certificate. Once that is completed, you will move on to the Coverages section of the form.
In almost every case, the certificate holder is the employing entity. Lets look at a quick example. Your general contracting company is working on a new building. You subcontract out the drywall installation and, as a matter of course, request a certificate of insurance from the drywall subcontractor.
