How to Write an Incident Report: A Step-by-Step Guide Step 1: Provide Fundamental Information. Step 2: Take Note of Any Damages and Injuries. Step 3: Identify Affected Individual(s) Step 4: Identify Witnesses and Take Their Statements. Step 5: Take Action. Step 6: Close Your Report.
Each event should be recorded and include: Workers name. Date and time of injury. Location and nature of the injury. Description of how the injury occurred. Type or description of first aid treatment given. Time first aid was given. Workers/Patients signature. First aid attendants signature.
First aid incidents should be recorded in a designated logbook or system provided by the employer or organisation.
Step 4: Complete the OSHA Form 300 Fill in the year, establishment name, city and state. Assign each event an employer-created case number on the OSHA log. Identify the employee, the employees job title, the date of the injury or onset of the illness and the exact location where the event occurred.
Useful information to record includes: The date, time and place of the incident. The name and job of the injured or ill person. Details of the injury/illness and what first aid was given. Details about what happened to the person immediately afterwards (e.g. went back to work, went home, went to hospital).
First aid can include cleaning minor cuts, scrapes, or scratches; treating a minor burn; applying bandages and dressings; the use of non-prescription medicine; draining blisters; removing debris from the eyes; massage; and drinking fluids to relieve heat stress.
The record must include: the injured persons name; the date and time of the injury; the location and nature of the injury; the time when first aid was given; the first aid treatment provided; the name of the person who provided first aid; and. the name of the person to whom the injured person reported the injury.
Complete an Injury and Illness Incident Report (OSHA Form 301) or equivalent form for each injury or illness recorded on this form. If youre not sure whether a case is recordable, call your local OSHA office for help. Feel free to use two lines for a single case if you need to.
