Today were going to talk about W-4s, W-2s, W-9s, and why the IRS has so many of these forms, and which one you should complete. A W-9, Request for Taxpayer Identification Number and Certification is the form which Americans use to give their Social Security Number (SSN) and other identifying information to a new employer. In Canada, Form TD1, Personal Tax Credits Return is a combination of the US equivalent to the W-9 and the W-4 form.
It should match the name on your individual tax return. If you have a business name, trade name, DBA name or disregarded entity name, fill it in here. If you do not have a business, you can leave this line blank. This section defines how you, the independent contractor, are classified when it comes to federal taxes.

The name on your return needs to match the name the IRS has on file for you usually the name printed on your Social Security card. Leaving out a middle initial or adding a middle initial when you didnt use one previously can create confusion.
