If the Social Security card contains a middle name, always complete the Form W-2 using just the middle initial (with no punctuation). You can enter a suffix; e.g., Jr., Sr., in location 62-65 of the Code RW, Employee Wage Record. The field for the middle name or initial is 15 spaces.
Make sure that the name shown on your Social Security card matches the name you use on your tax return. If you havent changed your name with the SSA, youll need to show your former name on the tax return instead of your married name to avoid delays.
IRS ITIN Pub 1915 for Line 1a says: Enter your legal name as it appears on your documents (again, so far, this sounds like a middle name on a W-7 and the letter is a MUST) but then states: this entry should reflect your name as it will appear on a United States Federal income tax return.
The components of a W-9 form include: Name: Write your full name, including your first, last and middle, if applicable. Make sure your name is identical to the name you write on other tax forms. Address: Include the street name, city and state where you currently reside, as well as the correct zip code.
When you legally change your name, there are tax consequences. The IRS wants people experiencing a name change to remember these important things: Notify the Social Security Administration of a name change ASAP. If a name on your tax return doesnt match SSA records, it can delay the IRS processing of your return.
The name on your return needs to match the name the IRS has on file for you usually the name printed on your Social Security card. Leaving out a middle initial or adding a middle initial when you didnt use one previously can create confusion.
A name control is a sequence of characters derived from a taxpayers name that is used by IRS in processing the tax return filed by the taxpayer.
