What information do I need for an ID Card? Please refer to our Required Documentation page for documentation needed to obtain a Utah state identification card. There is a $23.00 charge for an ID card.
Faculty, staff, students, sponsored affiliates and associates have a user ID known as Directory ID. This is the alphanumeric ID of up to eight characters obtained/created at account activation. The University Identification (UID) is the nine digit number typically found on the front of your University ID card.
Need an ID card? We make it easy. Just upload the photo you want for your UO ID card in advance, and once approved, we will have it ready for you to pick up in the Card Office.
