Theres a pretty wide range for how to make a proxy. Some people just write the name and abilities on a piece of paper covering a reversed card in a sleeve, some write the name of a card on the back of a bulk card like basic lands, and many people just print out the card in question.
No proxies will be allowed. This is the stance WOTC has on all sanctioned formats and we will follow those rules until they have been more cleary defined. The only time a proxy will be allowed is if, prior to the event, the player confirms he owns the card but doesnt want to have it in their deck.
A proxy card will almost always have a different rosette pattern than a genuine card. It can be one thats well done, but different nonetheless. When doing this test, its important to have a reference card from the same set to go back to, since genuine cards can have different patterns over the years.
Some of these proxy cards are so professionally made that they can pass as real Magic cards to the untrained eye, and are therefore effectively counterfeits. The production of these cards, for sale, is of course illegal.
Proxy cards are not legal for play in any sanctioned tournament but are often used in small testing groups to give players the opportunity to test a card without having that card in their collection.

Proxies arent legal in MTG, and depending on how theyre made, they sometimes skirt the line of legal in general. Counterfeit cards and proxy cards arent technically the same. There is a distinction, but it can be a bit of a grey area.
From the MtG tournament rules, section 3.3 Authorized Cards and 3.4 Proxy Cards, it is clear that only official, genuine cards are allowed in a tournament. A player can alter the card to a degree, as long as the card remains unmarked and essential information remains legible.
The most important distinction to make here is that proxy cards, no matter how realistic, are not legal in tournament magic. This means that in any official tournament, whether a Mythic Championship or just FNM at you local store, the cards are illegal to play with.
A Proxy is a card that is used to take the place of a real card. Proxies cannot be used in official tournaments. They are usually used when people are testing out a new decklist that maybe they found online or from a friend.
