but welcome to another MedCram lecture today were gonna talk about something a little different in terms of treatment we could actually talk about DNR full code palliative care just a quick primer on those sorts of things especially in terms of what we do in the intensive care unit but also what we do on the floor so I think this is kind of a confusing topic for some people who are not used to it DNR stands for do not resuscitate and of course the do and the not are pretty clear but the resuscitate can sometimes be a little confusing so when someone says somebody is DNR that means they dont want to be resuscitated but what does that exactly mean and I have they fully given consent about what it is that they want to have done well the opposite of DNR is a term that we sometimes call full code and that comes from the fact that when somebody in their heart stops or they stop breathing they have something called a code blue and its during that code blue that you can do all sorts of thi

Regardless of the format or the venue, DNR orders almost always follow the same general rules to be valid: DNR orders must be written by medical professionals rather than verbalized. Written DNR orders must include the patients name. DNR orders must be dated. The DNR order must be signed by the healthcare provider.
The DNR order is rescinded during surgery and the perioperative period and the patient consents to the use of any resuscitation procedure needed to treat the clinical events that occur. The original DNR order is maintained and prior treatment limitations are upheld.
Unlike a hospital DNR that does not expire, a non-hospital DNR must be renewed every 90 days by your physician.
A DNR order may be invalidated if the immediate cause of a respiratory or cardiac arrest is related to trauma or mechanical airway obstruction.
The DNR order is rescinded during surgery and the perioperative period and the patient consents to the use of any resuscitation procedure needed to treat the clinical events that occur.
DNR should not be automatically suspended before an operation, but there should be a discussion and a decision by the patient about intraoperative resuscitation preoperatively. Palliative operations can be justified in patients with a standing DNR.
A DNR (do not resuscitate) order is a request not to have CPR (cardiopulmonary resuscitation) if your heart stops or if you stop breathing while you are in a medical facility. An out-of-hospital DNR is for people who do not want to be resuscitated if they have problems at home or anywhere outside of a medical facility.
This policy focuses on patients who accept a surgeons recommendation to have surgery and who already have in place an advance directive, specifically, a Do Not Resuscitate (DNR) order. The best approach for these patients is a policy of required reconsideration of the existing DNR orders.
