Documentation in the patients medical record of a decision not to do CPR must include both a Physicians Order and an entry in the Progress Notes. Use of the standardized DNR Order Form is preferred, but is not required in cases where the attending physician prefers a narrative method of documentation.
In summary, the minimum requirement in order for a DNR form to be valid is the valid signature and date of the patient, or of their legally recognized healthcare agent in the event that they are incapacitated or otherwise unable to express their wishes.
Your DNR orders are in place for as long as you wish them to be; you need only to destroy them if you wish to stop them. You should also contact your doctors office and family if you do so.
How is a DNR Order Created? If you decide you want a DNR order, tell your doctor and health care team what you want. Your doctor must follow your wishes, or: Your doctor may transfer your care to a doctor who will carry out your wishes.
Your doctor makes the final decision. Doctors do not have to give you treatment if they think it will not work. A DNACPR decision must be made on an individual (person by person) basis. This means it must be based on your health, needs and priorities as an individual. You can use an advance directive form or tell your doctor that you dont want to be resuscitated. Your doctor will put the DNR order in your medical chart. Doctors and hospitals in all states accept DNR orders. They do not have to be part of a living will or other advance directive.
