Powers of attorney in New York Must be signed and dated by the principal. The principal must acknowledge the document before a notary public (required for the transferring of real property) Must be signed and dated by the agent in the presence of a notary public. Must be signed with two witnesses present.
Use the following pointers for how to sign using power of attorney to avoid any issues. Have a copy of the power of attorney document on hand when you sign anything on behalf of the principal. Print the principals name first on the signature line. After the principals name, write by and then sign your own name.

(NOTE: This power of attorney will not be effective unless it is signed by at least one witness and your signature is docHubd, using the form below. The notary may not also sign as a witness.)
To make a POA in Illinois, you must sign the POA in the presence a notary public and at least one witness. The notary public cannot act as the witness. Additionally, the following people cannot be your witness: your doctor or mental health service provider (or a relative of the doctor or provider)
Who signs a LPA? The LPA documents require signatures from not only the donor but also the attorneys. These signatures must be witnessed by one other person and the witness must be present, not watching the donor sign online or on a video call.
