this is dr daniel paul with easy orthopedics and today i want to talk about something in healthcare uh that not many people are talking about but has been bothering me for a long time and thats the fax machine so about twice a week i get a call hey dr paul uh we couldnt find your fax number whats your fax number to which i respond i dont have one and then theres usually a pause and then well how are we supposed to send your stuff i dont know how are you supposed to send me stuff how about you email it to me its 20 20 just email to me what are you going to send it send a fax so prints off on the computer and i have a bunch of papers im not dealing with that just email it to me send it electronically if ronald reagan were president and i just came out of an aerobics class and i was calling it from my car phone yeah then fax it to me but ronald reagans been dead for 16 years along with all the other stuff just emailed to me now some of you may be saying well you know i dont kno

You can contact them directly, toll-free at 1-800-387-0774. All claims are established through the Toronto office of the Workplace Safety and Insurance Board. To avoid delays, fax or mail completed Form 7s to the Toronto Office. Once the claim is established, the WSIB will send the employer the claim number.
Who completes it? The health professional (e.g., doctor, physiotherapist, or other licensed health care professional) who is treating the worker completes the form at the employers request or at the workers request. The health professional will provide the employer and/or the worker with the completed form.
