There are two methods to send questionnaires to patients. The first method is via the message center: click on "Messages," then "Send Questionnaire," enter the patient's name, select the questionnaires to send, and click "Send." The patient will receive an email notification detailing which questionnaires need to be completed along with a link to the Patient Health Record (PHR) login. The second method involves using the calendar during the new appointment process. Check the relevant questionnaires in the appointment pop-up window and click "Confirm" to finalize the appointment and send the questionnaires. The patient will be notified once the appointment is confirmed.
