Tips Write clearly and get straight to the issue. Soften the impact of bad news by using words like issue, challenge and difficulty rather than problem. Use positive words with a negation, e.g. not easy, rather than negative-sounding words, e.g. difficult.
How to write an incident report Prepare the basic facts. Explain the sequence of events that led to the incident. Analyze the incident or near miss. Describe what types of medical treatment occurred. Proofread and submit your report.
What information should be recorded? The date, time and place of the incident. The name and job of the injured or ill person. Details of the injury/illness and what first aid was given. Details about what happened to the person immediately afterwards (e.g. went back to work, went home, went to hospital).
Best practices to follow when writing an incident report Be objective when describing events. Dont editorialize or speculate about why something happened simply state what happened and how it affected you or others involved in the accident. Use simple language. Include photos, if possible.
