How is a DNR Order Created? If you decide you want a DNR order, tell your doctor and health care team what you want. Your doctor must follow your wishes, or: Your doctor may transfer your care to a doctor who will carry out your wishes.
There are currently two types of DNR orders: 1) DNR Comfort Care, and 2) DNR Comfort Care - Arrest. Upon the issuance of either order, standard forms of identification are provided for in OAC rule 3701-62-04.
A Do Not Resuscitate (DNR) order focuses only on a persons preference to not receive cardiopulmonary resuscitation if they stop breathing or their heart stops beating. The DNR does not address other medical treatments. A POST form typically covers preferences for a wider range of treatments, including CPR.
There are currently two types of DNR orders: 1) DNR Comfort Care, and 2) DNR Comfort Care - Arrest. Upon the issuance of either order, standard forms of identification are provided for in OAC rule 3701-62-04.
A DNR order does not mean do not treat. Rather, it means only that CPR will not be attempted. Other treatments (for example, antibiotic therapy, transfusions, dialysis, or use of a ventilator) that may prolong life can still be provided.
I, , request limited emergency care as herein described. I understand DNR means that if my heart stops beating or if I stop breathing, no medical procedure to restart breathing or heart functioning will be instituted.
Generally, a DNR order should not be entered into the patients medical record without concurrence of the parents. However, a DNR order may be entered by the patients physician without parental permission if CPR is determined to be physiologically futile or non-therapeutic, in ance with Paragraphs E and F below.
How can I write an advance directive? Use a form provided by your doctor. Write your wishes down by yourself. Call your health department or state department on aging to get a form. Call a lawyer. Use a computer software package for legal documents.
Establish the setting. Ensure comfort and privacy; sit down next to the patient. Ask if family members or others should be present. Introduce the subject with a phrase such as: Id like to talk with you about possible health care decisions in the future.
The attending physician/provider must provide the DNR/DNAR order, either in writing or verbally. A verbal DNR/DNAR order may be taken by a licensed nurse and co-signed by the physician/provider within 24 hours. Agreed to by the designated healthcare agent(s), for a patient without decision-making capacity.
